Q?Do I need to bring any forms with me?

I have a brief intake and disclosure form for you to fill out during our first session.

Q?What are your fees?
A.

I charge $125.00 per session. Payment is requested at the start of each session.  I accept cash, checks, credit cards, and HSA cards. If you need to cancel or reschedule please give me at least 24 hours of notice.  Missed sessions without prior notification will be charged the full amount. A sliding scale is available upon request.

Q?What are your Hours of Availability?
A.

I am available Monday through Thursday morning, afternoons, and evenings, and Saturday mornings and afternoons.

Q?Do you accept insurance?
A.

I am an out-of-network provider for insurance companies.  I request full payment at the beginning of each session and will provide you with a receipt.  You are responsible for submitting a claim to your insurance company and your coverage is determined by the plan that you have with your health insurance provider.

Q?What is your Therapeutic Orientation?
A.

My primary modality is Lifespan Integration, which gently heals trauma from both the body and the mind, and integrates the self through time.
